Emergency services reported that one man had suffered life-threatening injuries and was air-lifted to a hospital in Bristol after being cut free from his car.
The motorway was closed in both directions while crews worked to treat drivers for injuries and clear the debris.
A spokesperson for the Ambulance service described how the accident happened: "We received the first of several 999 calls around 5.15pm reporting road traffic collision on the M5.
"Callers reported a lorry traveling northbound just past junction 11 collided with several cars - at least some of which were traveling southbound."
Two other drivers were placed on spinal boards and another two were assessed at the scene by paramedics.
